All Athletes must have

Current Physical (physicals are only good for one calendar year)

Insurance Consent form (form stating you have insurance or would like to purchase a small policy)

Concussion form (information sheet about concussions

Copy of players Birth Certificate 

Athlete must complete the ENTIRE STAR sportsmanship program on www.starsportsmanship.com
